Our Nursing Process

Initial Nursing Assessment

Each case begins with a comprehensive in-home nursing assessment conducted by a licensed Registered Nurse. During this visit, the nurse evaluates the client’s health status, daily living needs, environment, and personal preferences.
The goal is to gain a full understanding of the individual’s physical, emotional, and social well-being — ensuring that care is both clinically sound and personally meaningful.

Personalized, Patient-Centered Care Plan

Following the assessment, the nurse creates a customized care plan tailored to the client’s unique needs, abilities, and goals.
This plan serves as a roadmap for the home health aide, detailing how to assist with personal care, mobility, nutrition, medication reminders, and other daily activities — while promoting independence and safety at home.

Ongoing RN Supervision & Support

Our Registered Nurses provide regular supervision of all home health aides, ensuring that care remains consistent, compassionate, and compliant with all New York State Department of Health standards.
Through scheduled visits, evaluations, and communication with families, the nurse ensures that each care plan remains up to date and continues to meet the client’s evolving needs.
