What You’ll Need

To help us streamline your application, please have the following documents ready:

  • Two forms of identification
  • A recent physical (including PPD and/or chest X-ray)
  • Proof of immunization for Rubella and Rubeola
  • Two professional references
  • NYS Certificate (for Home Health Aides/Personal Care Aides)
  • NYS License (for Registered Nurses)
  • If you don’t have NYS Certificate we provide FREE Training and Certification for Home Health Aides (HHA) and Personal Care Aides (PCA)
